Health Systems
Unlock Capital. Know Where to Grow.
Health systems rely on Montecito as a partner for growth, improved operations, debt reduction, and development. We monetize owned real estate without disrupting clinical control, develop outpatient facilities where patients are already going, and give leadership the full medical market picture, that fragmented data cannot give you confidence on your next move.
What We Hear
The Pressure on Health System Balance Sheets Has Never Been Greater.
“Our daily cash on hand is less than adequate for our bond rating.” “We don’t have the cash to expand; we need a funding partner.” Health systems face an imperative to grow at exactly the moment balance sheets are constrained by declining reimbursement and rising operating costs, and the shift to outpatient care is no longer a trend. It is the dominant growth model. The question is not whether to expand but where, when, and how, and how to fund it without carrying more real estate risk.
How We Help
Montecito Has Become an Indispensable Growth Partner for Health Systems Nationwide
Unlock the Capital in Your Real Estate
Selective monetization of owned medical office buildings, or an entire portfolio, through long-term net leases that preserve clinical control and operational flexibility. Capital moves off the balance sheet and into service lines, sites of care, physicians, and technology; liquidity and credit positioning improve; you keep full discretion over timing, scope, and structure.
Build Your Next Site Where the Data Says
Build-to-suit medical office and ASC development, sited with a North Star Growth Assessment of physician need, out-migration, and referral leakage, with up to 100% of project costs funded through Montecito’s discretionary capital and lender relationships, and flexible ownership structures that include physician and developer partners.
We Don’t Just Buy Your Buildings. We Show You Exactly Where to Build the Next Ones
Atlas integrates your own referral, utilization, and payer contract data with external market intelligence: where you are losing patients and why, which specialties to recruit, which locations constrain access, what a new site is worth at your actual payer mix, and how your reimbursement rates compare to competitors for the same services.
Atlas in Practice
A Seven-County Health System, a 136-Physician Deficit, and $298M in Identifiable Revenue
For a major health system operating across a seven-county region in the Southeast, Atlas analyzed provider supply and demand across 20 specialties. It found a current deficit of more than 136 full-time-equivalent physicians, forecast to exceed 152; 380,000 square feet of medical office space needed for the top ten specialties, with 65,000 available; and $298 million in estimated annual revenue those providers could generate in the identified growth areas. The system’s own patient-origin and referral data revealed ZIP codes with single-digit market share despite nearby facilities, millions in recoverable referral leakage, and specific contracts to renegotiate. Montecito became the system’s real estate partner for the expansion that followed.
